The Bosniak classification separates renal masses into 1 of 5 categories: 1, simple cyst; 2, minimally complex with single thin (1‐mm) septations, a thin calcified wall, or both (well‐marginated, nonenhancing, high‐attenuation renal lesions <3 cm are also included); 2F, increased number of septa, minimally thickened or enhancing septa or wall, or thick calcifications (hyperdense cysts Bosniak was the normal radiological assessment procedure for cystic renal lesions. Originally, it relied on computed tomography (CT) scan results and subsequently extended to magnetic resonance imaging (MRI). Renal cell carcinoma (RCC) surveillance was not enhanced in the past 20 years. In our study, one-half of small (< 4 cm) Bosniak category 3 and 4 cystic renal lesions were downgraded, and most (88%) of the small Bosniak category 2F lesions were downgraded or remained stable on follow-up. In none of the patients with lesions that were Bosniak category 3 or 4 at follow-up imaging did recurrent or metastatic disease develop.

No patient in this study developed metastatic disease within the three-year followup period. Smaller (<4 cm) Bosniak III cysts were more likely to be malignant and lesion size should be taken into consideration when considering management of complex cysts. The Bosniak classification is widely used by radiologists and urologists for addressing the clinical problem assessing renal cysts 3.

Last month a CT scan was done for a stomach problem and of that cyst showed again as complex cyst. I had my first scan on the 19/1/10 and it showed a 5.8 cm x 4.8 cm x 4.6 cm "Type 3 Bosniack cyst with enhanced matter"on the left kidney and just 4 weeks later on the 19/2/10 the next scan upgraded it to Type 4 and it was 6.2 cm x 5 cm x 5.2 cm. For patients with a solid or Bosniak 3/4 complex cystic renal mass in whom the risk/benefit analysis for treatment is equivocal and who prefer AS, physicians should repeat imaging in 3-6 months to assess for interval growth and may consider RMB for additional risk stratification. (Expert Opinion) my names sarah im 43 last week i was told by the hospital after many tests that i have a bosniak 3/4 complex cyst on my left kidney size 124x103x58mm suffer fom a lot of back and leg pain funny enough on the left side not the side of the tumor im very worried as been told moe than likley cancer and will probably remove the kidney any one had same of same size or bosniak size thanks Bosniak Classification of Renal Cystic Disease.
